Dear Parent or Guardian,
Due to recent legislation, specifically House Bill 241, parents must give written consent in order for their child to participate in any school health services. This is a big change. In previous years, health services were provided to all students and parents were given the option to opt out of health screenings. Only medication administration and medical procedures required written consent.
Parents will now be given the option to choose more specifically those services from health care staff that they wish to authorize for their child.
Health services include first aid, emergency care, health appraisals, nursing assessments, health referrals, health counseling, health promotion, disease and injury prevention, health education, and health screenings.
If you have not provided written consent for health services, we cannot provide any health assessments. You will need to pick up your student from school if they come to the clinic for any reason.
A consent form is available at your child's school and is available on the Health Services page of the District website under parent resources.
The Student Health Verification Form will be sent home during the first days of school. This form can also be utilized to give your written consent.
